Qualifications

• Minimum of two (2) years of LTSS and/or HCBS care coordination or care management experience.
• Experience with care delivery monitoring and service coordination.
• Strong assessment, communication, documentation, and organizational skills.
• Proficiency with computer systems and electronic documentation.

Responsibilities

• Coordinate comprehensive care management services for members receiving Long-Term Services and Supports.
• Develop and maintain individualized long-term service and care plans.
• Educate members and families on available healthcare benefits and community resources.
• Conduct member assessments through home visits and other approved locations.
