Job summary

A-Line Staffing Solutions seeks an experienced LTSS Service Care Manager to support North Carolina members with LTSS needs. This is a remote, field-based role with extensive travel for face-to-face visits across the service area.

Administrative work is remote, while in-field activities drive care coordination. The ideal candidate holds a Bachelor's degree with an active RN or LCSW license and 2–4+ years in physical healthcare case management, including hospital/discharge planning or utilization

Qualifications

• Bachelor's degree required.
• Active RN or LCSW license required.
• 2–4+ years of physical healthcare case management experience.
• Experience in hospital case management, utilization management, home health, or discharge planning.
• Valid driver's license required.
• Willingness to travel extensively for face-to-face visits.
• Strong Microsoft Office skills.
• Experience working with electronic medical records.

Responsibilities

• Manage a caseload of LTSS needs.
• Conduct comprehensive assessments with members, caregivers, and providers.
• Develop and monitor individualized service plans.
• Conduct monthly and quarterly member contacts.
• Complete face-to-face member visits in homes and community settings.
• Monitor delivery of authorized services and ensure continuity of care.
• Coordinate referrals and services based on member needs.
• Maintain accurate assessments, care plans, and case notes.
