Job summary

UnitedHealthcare seeks a LTSS case manager to support intake, screening, referral, assessment and ongoing case management for members in Colorado. You will document contacts, develop and update care plans, and monitor health and welfare outcomes in our prescribed systems.

Role requires travel up to 50% within the territory, confidentiality adherence, and collaboration in quarterly and annual assessments. A Bachelor's in human behavioral sciences or substantial LTSS experience is required.

Qualifications

• Must meet one of: Bachelor's degree in human behavioral science fields; 5+ years of LTSS experience; or a combination of education and LTSS experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Must reside locally to Alamosa, Rio Grande, Costilla, Conejos and Saguache Counties.
• Requires travel up to 50% within assigned territory.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management and documentation.
• Monitor health, welfare, and services; ensure confidentiality per policy; update care plans accordingly.
• Conduct quarterly and six‑month contacts and face‑to‑face reassessments; complete annual reassessments as needed.
• Oversee delivery of services in the support plan and PAR; participate in required CMA training.
• Travel between locations; comply with Colorado licensure and liability requirements.
