Job summary

UnitedHealthcare seeks a Case Manager to deliver Long Term Services and Supports case management in Colorado, including intake, assessments, development of care plans, and ongoing management. You will document in the required systems, attend supervisor conferences, and ensure confidentiality throughout the process.

The role requires local travel within Colorado, including Alamosa and nearby counties, with remote work flexibility as appropriate.

Qualifications

• Bachelor's degree in human services, nursing, social work, psychology or public health.
• 5+ years of LTSS experience or combination with education.
• 1+ years of MS Office experience.

Responsibilities

• Intake, assessment, and development of support plans; ongoing case management.
• Maintain confidential patient information and document activities per policy.
• Contact members quarterly or as needed; conduct face-to-face visits every six months.
• Update assessments and care plans in the documentation system.
• Monitor service delivery and adherence to PAR.
• Attend CMA agency trainings and internal staff development sessions.
• Travel within assigned territory; ensure transportation as required.
• Ensure work aligns with CMA Contract requirements.
• Refer suspected mistreatment to protective services or law enforcement as needed.
• Take on special projects and system assessments when requested.
• Focus on medical, behavioral, and social determinants affecting health.
