Centene Corporation in the United States is seeking a Care Manager to assist in developing, assessing, and coordinating holistic care management activities for long-term care members. You will educate members and families on available services and benefits, and help create personalized plans. The role requires residence in Sedgwick County or nearby, up to 50% local travel to visit members, with the option to work remotely the rest of the time.

Working hours are 8:00 a.m. to 5:00 p.m.

Formación

• Requires a Bachelor's degree and 2–4 years of related experience.
• Experience coordinating long-term care services is a plus.
• Provides education and resources to members and families.

Responsabilidades

• Evaluates the needs of the member and coordinates the care plan.
• Assists with developing long-term care plans and identifies providers and resources.
• Coordinates between member/family and care providers to ensure services are accessible.
• Provides resources to members and families for various needs (employment, housing, etc.).
• Monitors care plans and outcomes and makes recommendations.
• Interacts with long-term care providers to meet member needs.
• Collects and maintains member information to ensure regulatory compliance.
• May perform home visits to assess needs and collaborate with providers.
• Provides education to members and families on procedures and benefits.
• Provides feedback to leadership to improve quality and cost-effectiveness.
• Performs other duties as assigned.
• Complies with policies and standards.
