Qualifications

• Requires a Bachelor's degree and 2–4 years of related experience.
• Experience in health, human, social work, or education services with qualifying experience may substitute for a degree.
• HCBS waiver knowledge is preferred.

Responsibilities

• Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
• Develops or assists with personalized service/care plans for long-term care members and educates members and families on services and benefits.
• Coordinates between members, families, and care providers to ensure services are accessible and aligned with plans.
• Monitors plans and outcomes, providing recommendations to improve quality and efficiency.
