Centene Corporation is hiring a LTSS Service Care Manager to join our team. You will assist in developing and coordinating holistic care management activities to ensure quality, cost-effective outcomes for long-term care members.

The role involves field-based work, education for members and families, and collaboration with care teams to access services and resources. Requires a Bachelor's degree and 2–4 years of related experience.

Qualifications

• Requires a Bachelor's degree and 2–4 years of related experience.
• Or a Bachelor's in health, human, social work or education services with one year of qualifying experience; OR a high school degree with three years of qualifying case management experience.

Responsibilities

• Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
• Develops or assists with care plans/service plans for long-term care members and educates members and families on services and benefits.
• Evaluates member needs and resources to recommend the best plan for outcomes.
• Coordinates between member/family and care provider teams to ensure services are accessible.
• Provides resource support to members and families based on assessments.
• Monitors care plans and outcomes and offers recommendations based on needs.
• Performs home or site visits as needed to assess needs and collaborate with providers.
• Provides education to members and families on procedures, instructions, services, and referrals.
