Responsibilities

• Provide leadership to development and improvement of LTSS long-term care management process
• Set goals and oversee care management data & reporting metrics
• Lead policies and procedures to ensure compliance with standards
• Oversee caseloads and staffing of LTSS management team
• Monitor contract-required reporting and vendor oversight
• Stay updated on trends in payer care management and present process improvements
