Molina Healthcare is seeking a Care Manager to support LTSS coordination and care management for members in Wisconsin. You will work with a multidisciplinary team to develop and monitor care plans, including waiver services, and address psycho-social and medical barriers during member visits and remote interactions.

The role requires travel across Jefferson, Rock, and Walworth Counties with potential for remote work and collaboration with care teams to ensure high-quality, cost-effective member

Qualifications

• 2+ years health care experience, LTSS exposure preferred.
• 1 year care management or related medical/behavioral health setting.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and care coordination.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, and providers.
• Monitor care plan effectiveness and adjust as needed.
• Coordinate services across behavioral health and LTSS for continuity of care.
• Assess medical necessity and authorize waiver services; evaluate benefits and funding sources.
