Job summary

Qualifications

• At least 2 years of health care experience, including 1 year with LTSS or care management.
• Clinical licensure and/or certification as required by state contract or licensing mandates.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and may include in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and care network.
• Monitors care plan effectiveness and documents interventions and goals.
• Promotes integration of services across behavioral health and LTSS to enhance continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources; coordinates ICT meetings for approvals.
• Educates and motivates members using motivational interviewing and clinical guidelines.
• Identifies barriers to care and assists with psycho/social, financial and medical obstacles.
