Job summary

Molina Healthcare is seeking a Care Manager to support LTSS coordination and care management activities. The role collaborates with a multidisciplinary team to ensure high-need members receive integrated care across the continuum, with a focus on optimizing outcomes and reducing costs.

Responsibilities include conducting assessments, developing care plans, monitoring progress, and coordinating with providers and families. Travel across Jefferson, Rock, and Walworth counties is expected.

Qualifications

• At least 2 years of health care experience, including LTSS experience.
• 1 year of care management or equivalent in medical/behavioral health settings.
• Bachelor's degree may be required in some states.
• Valid and unrestricted driver's license for travel.
• Demonstrated knowledge of community resources.
• Ability to work in diverse settings with varied populations.
• Ability to work independently with minimal supervision.
• Strong written and verbal communication skills.
• MS Office proficiency and ability to navigate portals/databases.
• Experience with individuals with substance use disorders in some states.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and the care network.
• Monitors and documents progress toward goals; suggests plan changes.
• Promotes integration of LTSS and behavioral health services.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing and Molina clinical guidance.
• Identifies barriers and coordinates care to address obstacles.
• Collaborates with licensed care managers as needed.
• Travel 25-40% locally may be required.
