Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and LTSS activities, coordinating with a multidisciplinary team to deliver integrated member care. The role includes in-person home visits as required and travel within Jackson County, WI to visit members.

The ideal candidate has healthcare experience, care management background, strong communication skills, and the ability to work independently in diverse settings while using Microsoft Office tools to document progress

Qualifications

• Requires at least 2 years in health care with LTSS experience and 1 year in care management or equivalent
• Clinical licensure/certification only if state contract requires it
• Bachelor’s degree may be required in some states
• Valid driver's license and reliable transportation for travel requirements
• Experience with community resources and ability to work with diverse populations

Responsibilities

• Completes comprehensive member assessments, including in-person home visits
• Facilitates waiver enrollment/disenrollment processes
• Develops and implements care plans with the member and care network
• Monitors care plan effectiveness and documents progress
• Promotes integration of behavioral health and LTSS services
• Assesses medical necessity and authorizes waiver services
• Collaborates with interdisciplinary care teams for service decisions
• Educates and motivates members using motivational interviewing
• Identifies barriers and provides coordination to address psycho/social, financial, and medical obstacles
• Participates in travel to visit members within Jackson County, WI as required
