Job summary

Centene is seeking a fully remote Care Management Support role based in the United States. The position supports outreach, scheduling, and care plan education for LTSS Waiver members, with training and ongoing CT-based hours.

Preference is given to Illinois residents with healthcare admin experience. Responsibilities include member outreach, resource referrals, onboarding help, and non-clinical record maintenance, all while adhering to state and regulatory guidelines.

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.
• Pay Range: $17.84 - $28.02 per hour.

Responsibilities

• Outreach to members via phone to support care plan steps.
• Connect members to health plan and community resources.
• Serve as front-line support for member/provider inquiries.
• Assist onboarding and administrative duties, including welcome letters.
• Document and maintain non-clinical member records per policies.
• Advise on benefits and Social Determinants of Health resources.
• Perform additional duties as assigned and ensure policy compliance.
