• Experience with outbound calls to members or providers is preferred.

Responsibilities

• Provide outreach to members by phone to support with care plan steps and scheduling.
• Assist members in connecting to health plan and community resources for quality care.
• Serve as front-line support for member and provider inquiries and concerns.
• Assist with member onboarding and routine administrative duties including welcome letters and program materials.
• Document and maintain non-clinical member records per state and regulatory requirements.
• Identify Social Determinants of Health needs and make referrals.
• Perform other duties as assigned and comply with policies.
