Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence programs. You will conduct outreach, coach patients, and coordinate care with clinicians from a remote setting in the United States.

The role requires an active multi-state LPN/LVN license, 5+ years of clinical nursing, and comfort using AI-assisted tools while maintaining strong clinical judgment. Remote work with standard U.S. hours is offered.

Qualifications

Active LPN/LVN license in multi-state compact where you reside.
Minimum 5 years of clinical nursing experience required.
Remote work readiness with reliable high‑speed WiFi.
Active California LVN license required.
Spanish language skills preferred.
Experience educating and supporting patients with chronic conditions.
Experience with AI‑assisted clinical tools and ability to discuss AI vs. human judgment.

Responsibilities

Conduct outbound patient outreach and provide coaching and support for chronic conditions.
Help patients follow personalized care plans with education and motivation.
Perform comprehensive health assessments and readiness to change.
Educate on disease management, adherence, symptoms, and prevention per policy.
Monitor progress and outcomes via remote check-ins and tools.
Document interactions and connect patients with resources; escalate as needed.
Collaborate with clinicians using Cadence Platform, Google Workspace, and Slack.
