Job summary

Cadence is hiring a part-time Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients. You will conduct telephonic outreach, coaching, and care coordination, documenting interactions in the Cadence platform while collaborating with clinicians.

The role requires a multi-state license, 5+ years of clinical experience, remote readiness, and strong communication. Fluency in Spanish is a plus, and knowledge of AI-assisted clinical tools is encouraged.

Qualifications

• Active compact multi-state LPN/LVN license in the state of residence is required.
• At least 5 years of clinical experience as an LPN is required.
• Remote work readiness (stable high-speed WiFi) is required.
• An active CA state LVN license is required.
• Fluency in Spanish is preferred.
• Experience providing patient education and support to individuals with chronic conditions is preferred.
• Fluency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to discuss AI support vs. human judgment.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support for chronic conditions.
• Provide education and coaching on behavior modification, nutrition, physical activity, and self-management strategies.
• Conduct comprehensive assessments of health status and readiness to change.
• Educate on disease management, medication adherence, and prevention strategies; follow Cadence policies.
• Monitor progress and outcomes through regular check-ins and remote monitoring tools.
• Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
