Job summary

The Chart Desktop Management LPN is a full-time, remote clinical support role focused on owning and managing provider staff desktop workflows across assigned care teams. The position emphasizes timely follow-up, accurate documentation, and escalation of urgent items in alignment with LPN scope of practice.

Remote, NH-based candidates will collaborate with Nurse Manager and on-site teams to refine processes, maintain patient safety, and improve overall clinic flow as workflows evolve.

Qualifications

• Requires LPN licensure in New Hampshire and graduation from an approved practical nursing program.
• Demonstrates ability to document clearly and follow care plans within scope of practice.
• Experience with electronic health records and clinical messaging workflows is preferred.

Responsibilities

• Owns and monitors provider inboxes and clinical desktop work during scheduled hours.
• Prioritizes items by clinical urgency and routes as needed for review or escalation.
• Documents communications, test results, referrals, and orders in the EHR accurately and promptly.
• Escalates urgent or abnormal findings to licensed providers and ensures timely follow-up.
• Maintains remote-work productivity, confidentiality, and HIPAA compliance.
• Collaborates with Nurse Manager and care teams to optimize workflows and reduce backlog.
