Call4Health is seeking a licensed practical nurse (LPN) to join our care team in a remote capacity. You will support clients by managing Electronic Health Records (EHR) and patient portals, provide patient education, troubleshoot Remote Patient Monitoring (RPM) equipment and results, and counsel patients with chronic diseases between provider visits.

Qualifications

• LPN with 5+ years of full-time clinical experience, preferably in primary care for adults, pediatrics, and geriatrics.
• Experience using at least two electronic health record (EHR) systems.
• Active, unrestricted LPN license (e-NCL or Enhanced Nurse Licensure Compact); willingness to obtain additional licenses.

Responsabilités

• Monitor EHR inboxes and patient portal requests and respond per protocol.
• Refill prescriptions per protocol and coordinate follow-up care after discharge or procedures.
• Facilitate referrals and assist with prior authorizations requiring clinical input.
• Manage RPM results and educate patients on chronic disease management.
• Screen patients for clinical trials and conduct follow-up education.
• Support the healthcare team and communicate effectively with providers and patients.
