HealthSnap is seeking a motivated Care Navigator (LPN/LVN) for a full-time remote role in the United States. You will support patients in chronic care management and remote patient monitoring, with a NY nursing license required to practice.

Qualifications

• Current, valid New York Nursing License (LPN/LVN).
• Experience in cardiology or primary care settings preferred.
• Experience with remote monitoring and/or virtual care management programs preferred.

Responsibilities

• Conducts virtual consultations focusing on patient education for lifestyle and clinical status.
• Reviews remotely transmitted patient data (BP, glucose, weight, pulse oximetry) and documents in records.
• Educates patients on chronic diseases and supports setup of devices.
