Job summary

Call 4 Health is seeking an experienced Licensed Practical Nurse (LPN) for a fully remote position. The LPN will manage Electronic Health Records (EHR) and patient portals, educate patients, troubleshoot RPM equipment, and counsel patients with chronic diseases.

The role includes follow-ups between provider visits and medication refills within protocol and scope of practice. Responsibilities include coordinating care, processing refills, managing referrals, and ensuring clear communication among

Qualifications

Licensed Practical Nurse certificate required.
2-4 years of full-time LPN experience in primary care/settings preferred.
Experience using at least two electronic health record (EHR) systems.
Active multistate LPN license with eNLC; willingness to obtain additional licenses on request.

Responsibilities

Monitor and respond to EHR inboxes and patient portal requests per protocols.
Process prescription refills; coordinate follow-up care after discharge or surgery.
Facilitate referrals and prior authorization requests requiring clinical input.
Educate patients on RPM results and chronic disease management.
Screen for clinical trial eligibility and perform follow-up tasks.
Support healthcare team and enhance patient care delivery.
