Job summary

HealthSnap is hiring Michigan-licensed LPNs to support patients in chronic care management and remote patient monitoring. You will work with the patient’s care team to educate patients on chronic diseases like hypertension and diabetes, and empower them to manage their health.

This full-time role runs Monday–Friday, 9:00am–5:30pm ET, with state-specific pay. You will be trained on HealthSnap’s RPM platform and maintain compassionate, patient-centered communication.

Qualifications

• Current Michigan Nursing License (LPN/LVN) in good standing.
• Experience in primary care, cardiology, internal medicine, home care or CCM/RPM.

Responsibilities

• Conduct phone consultations and patient education for CCM/RPM programs.
• Educate patients on health conditions and motivate lifestyle changes.
• Maintain accurate, up-to-date patient records per protocol.
• Collaborate with care team to address patient needs and care plans.
• Respond to remotely transmitted patient data per partner workflows.
• Support continuous improvement initiatives within the navigator program.
