Job summary

HealthSnap is seeking New York-licensed LPNs to support patients in chronic care management and remote patient monitoring. This full-time, remote role operates Monday–Friday, 9:00am–5:30pm ET, collaborating with care teams to educate patients and empower them to manage their chronic conditions.

You will work with HealthSnap’s remote patient monitoring platform, maintain patient records, and communicate with providers following approved workflows.

Qualifications

• New York Nursing License (LPN/LVN) in good standing.
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management.
• Experience with EHR and care management software.
• Ability to work independently and as part of a team.
• HIPAA-compliant remote work setup and reliable internet.

Responsibilities

• Complete phone consultations and patient education for care management programs.
• Educate and empower patients to manage chronic conditions.
• Maintain accurate patient records and care plans.
• Address barriers to care and improve adherence.
• Communicate patient needs to providers per workflows.
• Participate in training and quality improvement initiatives.
• Respond to remotely transmitted patient data per workflows.
