Job summary

CareHarmony’s Care Coordinators (LPN) collaborate with providers to deliver value-based care management for patients. The LPN will identify resources, coordinate needs for chronic care management, and educate patients to improve health outcomes.

This fully remote, full-time role offers a stable weekday schedule with occasional on-call and opportunities for career growth within the company.

Qualifications

• Active Minnesota LPN license required.
• Minimum 3 years of direct patient-facing experience.
• Remote-friendly with ability to work from home.

Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in care.
• Identify and coordinate community resources for patients' care needs.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management, including reconciliation and refills coordination.
• Assist in ensuring timely delivery of services to patients (Home Health, DME, Home Infusion, etc.).
• Resolve patient questions and create open dialogue to understand needs.
• Assist/manage referrals and appointment scheduling.
