Qualifications

• Master's or Doctoral degree in Counseling, Psychology, or related field.
• Active, unrestricted LPC license in Arizona.
• At least two years of counseling experience in clinical settings.

Responsibilities

• Conduct comprehensive assessments to identify mental health needs.
• Maintain open communication with supervisors and family to monitor progress.
• Prepare and share appointment schedules and home visits.
• Coordinate home visits and in-home observations.
• Oversee implementation of treatment plans.
• Document monthly and quarterly progress notes.
• Lead training sessions for team members.
• Provide follow-up interviews and home visits as needed.
