Work Environment

This role is designed for counselors who thrive in a flexible, remote work setup, allowing you to craft your schedule while ensuring quality patient care. Although the position is primarily remote, you may be required to interact with clients through virtual home visits or in-person observations as needed. You will be part of a supportive team that leverages technology and evidence-based practices to enhance the therapeutic experience for our clients. Our outpatient office facilitates crucial team meetings and collaborative efforts to ensure high-quality service provision.

Key Responsibilities

• Conduct comprehensive assessments to identify the mental health needs of patients, ensuring tailored care strategies are developed and reviewed regularly.
• Maintain open lines of communication with supervisors, direct care staff, and family members of clients to monitor progress and adapt treatment plans as necessary.
• Prepare and share a tentative schedule of appointments and home visits, facilitating ongoing evaluation and adjustment of care strategies.
• Coordinate home visits and in-home observations to gather essential data for case reviews and improve client outcomes.
• Oversee the implementation of treatment plans, providing insights and recommendations to ensure their effectiveness.
• Participate actively in Interdisciplinary Team Meetings, contributing to holistic care discussions and planning.
• Document monthly and quarterly progress notes to track improvements and areas requiring further support.
• Organize and lead training sessions for team members to ensure competence in program implementation and documentation practices.
• Provide follow-up interviews and home visits as needed, reinforcing client support and continuity of care.
• Act as a liaison with the mental health community, enhancing resource availability and collaborative opportunities for patient development.
• Assist in the creation and refinement of training materials to elevate program effectiveness and professionalism across the team.

Patient Population / Client Focus

In this role, you will engage with a diverse range of clients, including adults and youth facing various mental health challenges. Our patients come from all walks of life and may experience conditions related to trauma, substance misuse, anxiety, and familial or relational issues. Your expertise will not only help individuals navigate their struggles but also empower them to achieve their personal and therapeutic goals.

Qualifications

• Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Current, active, and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.
• A minimum of two years of relevant counseling experience, ideally in clinical settings.
• Specialization in applicable areas such as family therapy, couples therapy, substance misuse, or trauma is highly regarded.
• Strong understanding of ethical and legal standards guiding mental health practice.
• Exceptional interpersonal and communication skills for building rapport with a wide range of clients.
• Proven ability to collaborate effectively with a multidisciplinary healthcare team.
• Proficiency with electronic medical records (EMR) software and related technologies essential for managing client data.

Preferred Experience

While two years of counseling experience are required, preference will be given to applicants with experience in multidisciplinary team environments and those who have demonstrated success in managing complex cases. Familiarity with telehealth practices is a plus, given the remote nature of our work.
