Job summary

Concert Health seeks a Collaborative Care Clinician II to deliver evidence-based behavioral health care via the Collaborative Care Model. The role supports pediatric and adult populations with mild to moderate symptoms and works with primary care partners to optimize outcomes.

Requirements include a Master’s in social work or related field, state licensure (IL or equivalent), 3–5 years licensed experience, and reliable internet.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• Licensed to practice in Illinois as an independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or equivalent).
• Minimum 3-5 years experience as a licensed professional.
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.

Responsibilities

• Provides care to patients based on Care Pod and customizes treatment plans.
• Coordinates behavioral health care with PCP and other providers.
• Screens patients for behavioral health and SUD disorders using PHQ9, GAD7, CSSRS, or other tools.
• Monitors symptoms and treatment side effects to reduce symptoms.
• Supports psychotropic medication management prescribed by PCPs.
• Tracks follow-up and outcomes using a registry and client records.
• Plans mutual self-management goals and actions.
• Documents progress in Salesforce and the EHR for sharing with care team.
• Participates in supervision and case consultations.
• Identifies social determinants barriers and connects to appropriate care.
