Job summary

Concert Health is hiring a Collaborative Care Clinician II to deliver evidence‑based behavioral health care using the Collaborative Care Model. The role serves pediatric and/or adult populations, coordinating with primary care and other providers to promote symptom reduction and optimal outcomes.

You will work 11:00am–7:00pm PT, contribute to care plans, and document progress in Salesforce and EHRs. A CA license and master’s in a related field are required.

Qualifications

• Master’s degree in social work, counseling, or related field.
• Licensed to practice in California as an associate clinician (ACSW/AMFT/APCC or equivalent).
• 1–5 years of experience as a licensed professional.
• High-speed internet capable of supporting video conferencing and cloud-based systems.

Responsibilities

• Provide care to patients based on Care Pod needs and customize treatment plans.
• Coordinate behavioral health care with the patient’s PCP and other providers.
• Screen patients for behavioral health and substance use disorders using PHQ9, GAD7, CSSRS, or similar tools.
