Job summary

Qualifications

• Master’s level degree in psychology or related field; Ph.D. or PsyD from an accredited college or university.
• Indiana licensure in LCAC, LMFT, LMHC or LCSW required.
• CPI certification must be obtained after hire and kept current.
• BLS and First Aid certifications must be obtained after hire and kept current.
• Valid driver’s license and active auto insurance.

Responsibilities

• Diagnose mental health and/or substance use disorders.
• Create treatment plans based on patient needs.
• Provide remote counseling and conduct ongoing assessments.
• Refer patients to other healthcare professionals as needed.
• Attend required supervision and staff meetings.
• Document services and use evidence-based practices.
