Job summary

Noble Healthcare Partners seeks a compassionate Licensed Professional Counselor (LPC) for a fully remote, full-time role providing personalized mental health services within a community health clinic framework. You will assess clients, collaborate with care teams, and maintain detailed treatment plans and progress notes.

Responsibilities include scheduling, coordinating home visits when needed, and participating in interdisciplinary team discussions.

Qualifications

• Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.
• Active LPC license in Texas.
• At least 2 years of counseling experience.
• Knowledge of legal and ethical standards pertaining to mental health practice.
• Excellent interpersonal and communication skills.
• Ability to collaborate within a healthcare team.
• Familiarity with EMR software.

Responsibilities

• Conduct thorough and ongoing assessments to evaluate mental health needs.
• Maintain communication with supervisors, care team members, and families about progress and treatment plans.
• Create tentative schedules for patient appointments and sessions.
• Coordinate and conduct home visits as needed to monitor progress.
• Ensure the program plan is executed effectively to meet client needs.
• Participate in Interdisciplinary Team Meetings to discuss cases.
• Complete monthly and quarterly progress notes.
• Organize and deliver staff training on program implementation and documentation.
• Provide follow-up support through interviews and home visits.
• Maintain accurate, accessible client records for care delivery.
• Act as liaison between mental health community and public health initiatives.
• Contribute to developing training materials and procedures.
