Job summary

CVS Health is seeking a Lead Director of Digital Product for Chronic Condition Management. Remote option with NYC area focus, reporting to the Executive Director, Medical Cost Initiatives.

You will design and execute technology roadmaps for chronic care programs, collaborating with Epic analysts, software engineers, data scientists, and clinicians to improve patient outcomes and reduce costs. Qualifications

Executive presence to communicate with senior executives and frontline staff.
Hands-on experience with Epic EHR workflows and configurations.
Deep familiarity with Value-Based Care constructs and FFS dynamics.
Proven track record leading cross-functional teams to deliver high-scale clinical interventions.

Responsibilities

Lead cross-functional teams to build end-to-end clinical technology pathways.
Translate clinical ideas into structured problem statements and Epic build specs.
Own product roadmap, backlogs, and ensure transparency with leadership.
Integrate external data sources with Epic workflows to provide real-time insights.
