VIM Medicine is seeking a Lead Care Coordinator to join our remote care team serving the East and Central U.S. You will work directly with physicians and patients to manage intake, scheduling, and timely communication across multiple cities.

As the second coordinator, you will help design playbooks, build workflows, and grow the team while ensuring each patient’s journey is followed from start to finish. We value proactive problem‑solving, privacy, and a startup mindset.

Qualifications

• Direct healthcare experience in care coordination, case management, transitions of care, or clinical operations.
• Ability to move quickly, verify details, and solve problems without a fixed playbook.
• Comfort coordinating with hospitals, labs, and multiple specialty practices.
• Experience with AI-enabled tools or systems is a plus.
• Strong communication and collaboration with physicians and patients.

Responsibilities

• Own the patient journey from intake to scheduling and follow‑ups.
• Close the loop on results and transitions of care.
• Build fast, reliable clinical logistics workflows (prescriptions, orders, authorizations).
• Lead standing panel reviews with physicians as the team scales.
• Define playbooks and escalation paths for the care coordination team.
