Vynca Inc. is seeking a Lead Care Manager to coordinate care for clients across health providers, social services and families. This role reports to ECM leaders and emphasizes person-centered care, accessibility, and cost-efficiency, with remote options and field-based visits as needed.

The LCM will build care plans, promote wellness, and ensure timely services while maintaining EMR records. Candidates should have 2+ years in care management, strong communication, and bilingual skills; a Pacific

Competenze

• 2+ years experience as a care manager, care navigator, or community health worker serving vulnerable populations.
• Willing and able to work Monday–Friday 8:30am–5:00pm Pacific Time with flexibility for evenings/weekends.
• Knowledge of government and community resources related to social determinants of health.
• Clean driving record and reliable transportation.
• Excellent oral and written communication and interpersonal skills.
• Proficient computer skills including Google Workspace and MS Office; internet.

Mansioni

• Assess member needs across physical and mental health, SUD, social supports, and housing.
• Develop and update client care plans and SMART goals.
• Provide care where the member resides: office, telehealth, or field-based.
• Link clients to social services and supports as needed.
• Advocate with health care professionals on behalf of the client.
• Apply Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
• Document progress in CRM and care plans; complete monthly reporting.
• Attend field trainings and ensure program compliance.
