Job summary

Integrated Behavioral Health Counselor – Remote requires compassionate clinicians to provide counseling and rehab services to diverse communities from a San Jose base. Roles include risk assessments, treatment planning, and collaboration with a multidisciplinary team, with a focus on integration into primary care.

Candidates should hold a Master’s in social work or psychology and CA licensure; 1+ year IBH experience; English fluency; bilingual skills desirable.

Qualifications

• Master’s degree in social work or psychology.
• Active California license or registration in relevant field.
• ≥1 year clinical Integrated Behavioral Health experience.
• Experience in healthcare setting valued.
• Knowledge of DSM‑V criteria.
• Proficient with electronic health records.
• Familiarity with ethics and legal standards in care.
• IMPACT Model knowledge is a plus.
• Excellent English communication; bilingual desirable (Spanish/Mandarin/Vietnamese).

Responsibilities

• Offer culturally sensitive, client‑centered counseling in individual, group, and family formats.
• Perform psychosocial assessments and craft personalized treatment plans.
• Refer to community resources and coordinate with case management.
• Provide crisis intervention for acute behavioral health needs.
• Collaborate with primary care teams to integrate behavioral health care.
• Document clinical records in compliance with standards.
• Participate in supervision, meetings, and ongoing training.
• Maintain LPS 5150 Authorization certification and serve as OD support as needed.
• Build effective relationships within a diverse team.
• Perform additional duties and projects as assigned.
