You will oversee insurance verification, authorizations, and coordination of care with multiple providers, while building strong relationships with referral sources and families.

Qualifications

• Healthcare intake experience in a home health or hospice setting.
• Nursing background preferred but not required.
• Excellent communication, negotiation, and public relations skills.
• Autonomy, assertiveness, flexibility, and cooperation in duties.
• Experience with Home Care Home Base is a plus.

Responsibilities

• Direct daily patient referral and intake operations, including policy development.
• Oversee insurance verification and authorization processes to maximize reimbursement.
• Establish and maintain relationships with current and prospective referral sources.
• Coordinate patient education and plan of care initiation with multiple providers.
• Ensure compliance with state, federal, and Joint Commission requirements.
