Qualifications

• High school diploma or equivalent is required.
• Two years in health care and one year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills including operating a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
