Job summary

Innovive Health is seeking an Intake Coordinator to manage end-to-end home health referrals. You will be a key contact between referral sources, patients, clinicians and internal teams, handling high-volume communications, verifying insurance, collecting documentation and entering patient data to ensure timely, compliant care coordination.

Hours are remote with Friday–Sunday shifts 9 am–5 pm EST; must reside in listed states.

Qualifications

• Associate’s degree or equivalent in related field.
• 1+ year experience in a healthcare administrative role.
• Knowledge of medical terminology.
• Proficiency in Windows, Word and Excel.

Responsibilities

• Answers all incoming calls including intake of new referrals, resumptions of care and readmissions for home health services.
• Directs other inquiries to the appropriate person or department.
• Collects demographic information and checks insurance eligibility.
• Enters patient data into referral tracking tools and EMR systems.
• Performs outbound calls to referral sources, providers and facilities.
• Collaborates with internal clinicians to ensure timely care needs.
• Partners with multiple departments to coordinate care.
• Maintains HIPAA compliance and protects patient confidentiality.
