Job summary

Abound Health is seeking an Intake Coordinator for a fully remote role supporting East Coast hours. You will report to the Intake Manager and serve as the first contact for families entering services, setting a welcoming tone and ensuring they feel supported as they begin treatment.

You will manage referrals, collect and review intake information, coordinate documentation, and collaborate with Program Directors to ensure a smooth intake and handoff to clinicians.

Qualifications

• Master's degree in a related human services field with a minimum of 1 year of full-time IDD experience.
• Bachelor's degree in a related human services field with a minimum of 2 years of full-time IDD experience.
• Bachelor's degree in an unrelated field with a minimum of 4 years of full-time IDD experience.
• Experience must include professional support to individuals with Intellectual and Developmental Disabilities.

Responsibilities

• Respond to questions and referrals from assigned counties, managing multiple referrals.
• Collect and review intake information to assess service appropriateness and urgency.
• Maintain regular communication with referral sources to support timely intake progression.
• Assist with service paperwork and upload intake documentation in Dynamics and OnTarget.
• Coordinate intake services with Program Directors and other teams.
