Job summary

Cognizant is seeking an Inpatient Coder (CCS Certified) for a remote role. You will review medical records, assign ICD-10-CM/ICD-10-PCS codes, and query physicians to clarify documentation. The position requires independence, teamwork, and adherence to accuracy and productivity standards in a remote environment.

Key duties include coding, peer reviews, reporting, and ensuring a minimum 98% accuracy rate. Epic experience and MS Office proficiency are required.

Qualifications

• Active CCS, RHIT, RHIA, CPC, or CIC credential.
• Three years of facility-based inpatient coding experience.
• Level 1 Trauma coding experience; Epic experience.

Responsibilities

• Review documentation to code ICD-10-CM diagnoses and ICD-10-PCS procedures.
• Query physicians to clarify diagnoses and procedures.
• Assign accurate codes using encoder applications per policy and regulatory guidelines.
• Perform peer reviews of HIM coders.
• Complete reports as requested.
• Maintain a minimum accuracy rate of 98%.
• Meet inpatient productivity: 16 encounters per day, 2 per hour, 30 minutes per encounter.
• Work across multiple systems.
