Job summary

Cognizant is seeking an Inpatient Coder (CCS Certified) for a remote role. You will review documentation, assign ICD-10-CM/PCS codes, and query physicians to clarify diagnoses and procedures.

With 3+ years of facility-based inpatient coding experience and Epic proficiency, you will maintain high accuracy and productivity in a fast-paced remote environment while supporting Cognizant clients.

Qualifications

• Active CCS, RHIT, RHIA, CPC or CIC credential.
• Three years facility-based inpatient coding experience.
• Level 1 Trauma coding experience required.
• Epic experience required.
• Knowledge of federal, state, and payer regulations.
• Ability to interpret medical records and chart entries.
• Strong judgment and decision-making abilities.
• Ability to work independently and in a team.
• Proficiency with MS Office.

Responsibilities

• Review documentation to code diagnoses (ICD-10-CM) and procedures (ICD-10-PCS).
• Query physicians to clarify diagnoses and procedures.
• Assign accurate codes using an encoder per policy and guidelines.
• Perform peer reviews of coding auditors and coders.
• Complete reports as requested.
• Maintain a minimum accuracy rate of 98%.
• Meet inpatient productivity: 16 encounters/day, 2/hr, 30 min/encounter.
• Work across multiple systems simultaneously.
• Coordinate with manager and team on time off and policy changes.
• Perform other duties as assigned.
