Signify Health, part of CVS Health, is seeking a Full-Time In-Home Physician to provide evaluations at patients' homes in the Cleveland, OH area. The role involves performing comprehensive health assessments, reviewing medications, and coordinating care with health plans.

Physicians will visit members in their homes (remote work options available) and may travel locally across licensed states, with a total weekly commitment of about 40 hours and comprehensive benefits for full-time staff.

Formación

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsabilidades

• Deliver in-home health evaluations for plan members.
• Review medications and medical history with patients.
• Perform physical evaluations and required diagnostic tests.
• Travel locally within licensed states as needed; conduct virtual visits when appropriate.
