You will review medical histories, perform clinical assessments, and coordinate with health plans to ensure appropriate care. The role includes travel and occasional virtual visits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations with patients and families.
• Review medication and medical history; perform physical evaluation.
• Travel locally and within licensed states; conduct virtual visits when needed.
• Provide Diagnostic & Preventive Services as indicated by health plan.
• Engage with health plan teams to coordinate appropriate care.
