Job summary

Monogram Health is seeking a full-time Palliative Care Advanced Practice Provider to deliver in-home palliative care to complex patients across the community. The APP will work in-market with the Market Physician Executive and Palliative Lead, performing assessments, symptom management, care planning, and advance directives.

Remote work with periodic trainings outside the home state is required. Eligible candidates hold NP or PA licensure with board certification and a DEA certificate; travel

Qualifications

• Active and unrestricted NP or PA license.
• Board certification for NP (ANCC/AANP) or PA (NCCPA).
• Current and unrestricted DEA certificate.
• Minimum of two (2) years in a palliative care setting.
• Ability to work autonomously without direct supervision.
• Willingness to travel within the designated region to meet patient needs.
• ACHPN or CHPN certification preferred.
• Experience with EHR and complex patient care.

Responsibilities

• Partner with leaders to identify eligible patients and develop individualized care plans.
• Deliver in-home palliative care visits and manage symptoms and treatment planning.
• Prescribe medications and coordinate tests in collaboration with pharmacies and Monogram physicians.
• Conduct follow-up evaluations and adjust plans as needed.
• Educate patients and families on disease progression and advance care planning.
• Lead discussions on prognosis and goals of care, including AD/POLST documentation.
• Coordinate transitions to hospice when appropriate.
• Collaborate with PCPs, specialists, and interdisciplinary teams, including chaplain support.
• Participate in team and clinical leadership meetings to drive KPIs.
• Close care gaps with preventive monitoring and diagnostic scheduling.
