Job summary

Community Housing Network (CHN) is seeking a Housing Stability Coordinator to provide strength-based housing case management and participate in the coordinated entry system. You will engage with program participants, partners, and the team to promote housing stability and outcomes.

The role includes field-work, in-office duties, and remote work, with occasional evening or weekend hours. A driver’s license and experience with housing services are preferred.

Qualifications

• Experience in case management or related field preferred.
• Strong communication skills with vulnerable populations.
• Bachelor's degree in social work or human services related field or two years of related experience.
• Demonstrated ability to work independently and collaboratively in a fast-paced environment.
• Working knowledge of Coordinated Entry System and related housing resources.
• Proficient computer skills; organized, proactive, and detail-oriented.
• Valid driver’s license and auto insurance minimums.
• Commitment to confidentiality and organizational values.

Responsibilities

• Meet program participants to complete paperwork and assess grant resources.
• Provide housing case management using strength-based approaches.
• Develop individualized housing stability plans with participants.
• Explore long-term affordable housing opportunities.
• Conduct monthly home visits to review goals and eligibility.
• Document case notes in HMIS per requirements.
• Link participants to community resources as needed.
• Represent CHN in community forums and events; may require evening/weekend hours.
• Stay informed on best practices through trainings and events.
• Navigate participant support and emergency needs as required.
