Job summary

The Coordinating Center is seeking a Homeless Support Services Coordinator to join the Housing Initiative Program. The role focuses on delivering person-centered services, coordinating housing supports, and promoting long-term stability for formerly homeless individuals receiving permanent supported housing services.

You will work under the HIP contract, maintain HMIS documentation, and collaborate with CES and community partners to engage recipients in housing stability efforts.

Qualifications

• Bachelor’s degree in human services is required.
• Two years of case management experience is required.
• Ability to travel throughout Montgomery County and conduct home and community visits is required.

Responsibilities

• Coordinate housing and supportive services using Housing First principles.
• Engage with program recipients to promote housing stability and recovery.
• Maintain documentation in HMIS and other databases.
• Collaborate with CES, HIP Housing Locator, and Community Health Nurse as applicable.
• Attend CES meetings and trainings to enhance best-practice knowledge.
