Ecumen Hospice and Home Care is hiring an Intake Specialist in Shoreview, MN. The role is part-time, 8am-4:30pm every other weekend, with a remote opportunity and on-site training for the first two months.

• GED or equivalent.
• Two years of Medicare Home Care/Hospice experience.
• Current valid driver's license and proof of vehicle insurance.
• Ability to communicate effectively.
• Able to follow all safety rules, regulations, policies and procedures.

Responsibilities

• Coordinates the intake of patient information to ensure timely admissions, treatment, interventions and resource utilization in compliance with policies, procedures and regulatory requirements.
• Verifies patient payer sources and schedules all hospice and home care admissions in software system and ensures accuracy of information.
• Responsible for coordination of all patient care; coordinates care and services with other departments/providers from the time of referral to discharge.
• Assumes added responsibility for special projects, administrative functions as directed and follows through with timely completion of the project.
