Job summary

Ecumen is seeking a Hospice Social Worker (PRN/Casual) to provide psychosocial support to hospice patients and families in Minnesota, with days 8am-4:30pm and no weekends or holidays. The role covers Duluth and North Branch teams and involves comprehensive social work services and end-of-life care planning.

Requirements include a Master’s in social work, Minnesota LGSW, and 2+ years in healthcare social work, plus a valid driver’s license.

Qualifications

• Master's Degree in social work is required.
• Licensed Graduate Social Worker in Minnesota is required.
• Two or more years of social work experience in a health care setting is required.
• Valid Driver's License and satisfactory motor vehicle record are required.

Responsibilities

• Assess psychosocial status of terminally ill patients and families; provide resources, counseling, and end-of-life education.
• Assess family/caregiver bereavement risk and provide resources/interventions.
• Monitor family dynamics and safety; develop interventions as warranted.
• Collaborate on care planning; attend interdisciplinary meetings; maintain clinical records.
• Assist with funeral planning; provide resources for financial, legal, and health care decisions.
• Facilitate care conferences and bereavement follow-up per protocol.
• Coordinate Mosaic Moments and hospice placements as needed.
