Job summary

Houston Hospice is seeking a part-time weekend Intake Specialist to join our Intake Team in Houston, TX. The role supports Saturdays and Sundays from 8:30 am to 5:00 pm, handling referrals, information requests, and admissions to ensure timely hospice services.

This position emphasizes accurate communication with referral sources, maintaining compliance with regulatory requirements, and building strong relationships with IDG and staff.

Qualifications

• Minimum one year of experience in the healthcare field
• One year of experience in a similar field
• Detail oriented
• Knowledge of applicable Federal/State laws governing home health and hospice licensure
• Excellent interpersonal skills
• Excellent communication skills, both oral and written
• Excellent organizational skills, ability to handle a variety of tasks simultaneously
• Reliable transportation; valid Texas Driver's License and Liability insurance

Responsibilities

• Communicate accurate information in a professional manner to referral sources
• Receive and forward referrals for appropriate follow-up.
• Request/obtain appropriate information, medical records, and physician orders on hospice referrals
• Participate in the QA/PI program, as requested
• Assure compliance with regulatory bodies
• Maintain the current admission referral board
• Submit information for verification of reimbursement source
• Assist in the development of intake/admission policies and processes
• Develop and promote positive relationships with IDG and other staff
• Communicate with supervisor if unable to complete a referral and assign admission staff
• Monitor the status of referrals on an ongoing basis
