Job summary

Alliance Health seeks a Care Manager for High-Risk Pregnancy to deliver comprehensive care management for eligible pregnant members in a hybrid role. You will identify risks, plan care, coordinate with OB and other providers, and connect members to needed social resources.

The role requires a relevant master's degree and NC licensure (LCSW/LMFT/LCAS/LCMHC/LPA) or RN with substantial post‑degree experience. Travel within Mecklenburg County may be required.

Qualifications

• Master’s degree and post-graduate experience in care management or related field.
• Active North Carolina licensure in one or more required disciplines.
• Experience in care management, maternal health or high‑risk pregnancy preferred.
• Ability to manage multiple priorities and collaborate with providers.

Responsibilities

• Provide CMHRP services to pregnant/postpartum members using whole-person approach.
• Develop and maintain individualized, member-centered care plans with care team.
• Coordinate with OB, primary/specialty providers, and community partners.
• Educate members on prenatal/postpartum care and available resources; address barriers.
• Identify and resolve transportation, housing, language, and other social needs.
• Escalate urgent concerns per approved workflows; document outreach and assessments.
• Coordinate reviews, training, and quality improvement activities; travel as needed.
