Job summary

Alliance Health in North Carolina is seeking a Care Manager for the Care Management for High-Risk Pregnancy program. The role covers identifying needs, conducting assessments, creating care plans, and coordinating with providers to support high-risk pregnant members.

The position offers a hybrid work arrangement with onsite meetings as needed and potential travel within Mecklenburg County. A Master’s in Human Services or an NC RN license with relevant experience is required.

Qualifications

• Master’s degree in Human Services and at least two years of full-time, post-graduate experience in care management or related field.
• Must be licensed in NC as LCSW/LMFT/LCAS/LCMHC/LPA or be a Registered Nurse with NC RN license and relevant experience.

Responsibilities

• Provide care team support for CMHRP during pregnancy and postpartum.
• Develop and maintain individualized, member-centered care plans with providers and supports.
• Conduct timely outreach and verify eligibility, enrollment, and documentation.
• Educate members on prenatal care engagement, postpartum care, and available resources.
• Identify and help resolve barriers to care (transportation, housing, language, etc.).
• Coordinate with OB, behavioral health, hospitals, and community partners.
• Document all outreach, assessments, plans, referrals, and communications.
