Job summary

Alliance Health is seeking a Care Manager for the Care Management for High-Risk Pregnancy (CMHRP) program. The role delivers comprehensive, member-centered care management to eligible high-risk pregnant members, coordinating with providers and community resources.

This full-time hybrid position supports early identification, outreach, assessments, care planning, and documentation in line with NC requirements. Travel within Wake, Durham, and Orange counties may be required.

Qualifications

• Master’s degree in Human Services or related field with 2+ years in care management.
• Must be licensed in North Carolina as LCSW/LMFT/LCAS/LCMHC/LPA or as RN with 3+ years post-degree experience.
• RN licensure must be active in NC.

Responsibilities

• Provide member-centered CMHRP care management during pregnancy and postpartum.
• Develop individualized care plans and coordinate with providers and community partners.
• Engage with members to address barriers to prenatal care and social needs.
• Document outreach, assessments, and care actions in the care management platform.
• Escalate urgent maternal health or behavioral health concerns per workflows.
